Our Community Link Worker team are busy and working hard to support Inverclyde residents
Our Community Link Worker (CLW) team, based in all 13 GP practices in Inverclyde, continue to work in a range of ways to support our patients. Like many, we continue to adapt to suit current guidelines and patient needs. While much of our interaction is telephone-based and through virtual platforms, we are attending our allocated GP practices each week and are also seeing patients in the community, often offering Walk & Talks when patients are able to do this (Inverclyde weather permitting!).
We continue to enhance our knowledge through conversations with our partners across statutory and 3rd sector organisations. We hold regular service update sessions where other agencies can provide information to our team. This keeps us up to date with information about what is currently available for our patients to enhance mental and physical wellbeing and help keep them connected to their communities.
Our team also undertake regular training across a range of relevant topics for the role. One of our team, Brenda Cox, has recently successfully completed the PDA in Technology Enabled Care – a challenging course but very relevant to the current times – well done Brenda!
Patients who feel they can benefit from Community Link Worker support for non-medical needs can access their allocated CLW by speaking with their GP practice staff.
